If you Lose A Household Member - Divorce

This checklist is provided as a concise summary of the many actions, forms, contacts, and decisions that you may need to be aware of--as you transition through this "life event". This list has been developed through our many years of assisting employees in similar circumstances. Most of the actions are optional-but worth considering. If you have any questions about special circumstances, suggestions for improvement, or lessons-learned please let us know.

Change FEGLI Life Insurance.
If you are enrolled in Basic, you may add or subtract Option B (1 x your salary for each dependent child)

add or subtract Option C -(up to 5x $2,500) Family coverage		 Within 60 days Use form:
