Passport Applications

To initiate a first time passport application, employees must complete an original Department of State passport application form (DSP-11: Application for Passport).   When renewing a passport application, employees must complete an original Department of State passport application form (DSP-82: Application For Passport By Mail). Separate application forms are required for dependents of employees who are permanently or temporarily changing stations (PCS/TCS) to an overseas station.

Passport applications may be picked up from the JSC International Travel Office located in Room 126 of Building 1. Completed passport application(s), must be submitted to the JSC Passport Agent. To ensure your passport(s) is processed in a timely manner, allow 3 weeks for processing.
